Ford Uses Sync To Manage Illness On The Go

In the last few years Americans have begun to take a more active role in managing their own health and well-being. Recognizing this trend, Ford researches are working on developing a series of health and wellness in-car connectivity solutions. These solutions are designed to empower people with self-help information while they drive.

Ford is using SYNC s ability to connect devices via Bluetooth, access cloud-based Internet services and control smartphone apps. They are developing voice-controlled in-car connections to an array of health aids including glucose monitoring devices, diabetes management services, Web-based allergen alert solutions and asthma management tools.

Ford SYNC is well known in the industry and with consumers as a successful in-car infotainment system, but we want to broaden the paradigm, transforming SYNC into a tool that can help improve people s lives as well as the driving experience, said Paul Mascarenas, chief technology officer and vice president, Ford Research and Innovation.

Constant Connections

Taking a smart, high-volume approach to bringing mobile health and wellness solutions inside the car, To develop some of the initial health and wellness programs Ford is using a high-volume approach; looking at two populations with the most need for constant connection to potentially life-saving information people with diabetes and those who suffer from allergies or have asthma.

According to the American Diabetes Association, nearly 26 million adults and children are currently living with diabetes in the U.S., that s over 3 million more than there were four years ago. The Asthma and Allergy Foundation of America reports that over 60 million Americans have asthma and/or have allergies.

Ford is working with experts in these fields to create relevant in-car features and services for those living with these ailments.

Ford SYNC offers three unique ways to bring health and wellness connected services into the car:

Device connectivity via Bluetooth Leveraging Bluetooth, medical devices can be connected to the car to share information through SYNC; just like a driver can connect and access their cell phone or address book by voice control

Cloud-based services Ford s off-board network of location-based traffic, directions, and information providers that drivers can access through their cell phones. Known as SYNC Services, new services such as medical services can be easily added through this plug-and-play voice-controlled capability

AppLink this innovation allows smartphone apps to be accessed by drivers via voice commands. The SYNC application programming interface (API) allows app developers to enable their apps to communicate through SYNC, delivering a smarter way for drivers to manage apps while driving

The ongoing Ford health and wellness research projects include:

Glucose monitoring: A prototype system that allows Ford SYNC to connect via Bluetooth to a continuous glucose monitoring device and share glucose levels and trends through audio and a center stack display and provide secondary alerts if levels are too low.

Allergy alerts: Ford is working with SDI Health and www.pollen.com to SYNC-enable its smartphone Allergy Alert app through AppLink, giving users voice-controlled access to the app that provides location-based day-by-day index levels for pollen; asthma, cold and cough and ultraviolet sensitivity; as well as four-day forecasts.

WellDoc: Ford and WellDoc, a recognized leader in the emerging field of mHealth integrated services, have joined forces to integrate in-car accessibility to WellDoc s comprehensive cloud-based personalized solutions for those with asthma and diabetes through SYNC Services. Using voice commands, SYNC users could access and update their WellDoc profile to receive real-time patient coaching, behavioral education and medication adherence support based on their historic and current disease information.

Burris sworn in as US Senator for Illinois

American politician Roland Burris of the United States Democratic Party was sworn in Thursday as US president-elect Barack Obama’s replacement in the United States Senate.

Burris was initially not admitted into the Senate, even though Governor of Illinois Rod Blagojevich appointed him to the position, due to allegations that the governor tried to sell the seat. Blagojevich was arrested and has been impeached for the alleged crime. Burris was denied his position on January 6 by other Democratic members of the Congress because his appointment papers were not signed by Jesse White, the Illinois Secretary of State, who had refused to sign his name due to the charges faced by Blagojevich.

Burris was officially sworn in by United States Vice President Dick Cheney with no objections from other members of the Congress. Harry Reid, the senior Democratic senator from Nevada and majority leader, who had led the opposition to Burris being appointed, welcomed him into the Senate.

“On behalf of all senators, Democrats and Republicans, we welcome you as a colleague and a friend,” Reid said.

“No one’s ever had anything against Sen. Burris. Like any other senator, he’ll have to learn the ropes and not get lost getting to committee hearings, like I have. I think he’ll come in with people open to meeting with him and getting to know him. I think it will be just fine,” said Senator Amy Klobuchar of Minnesota on the matter. Most other comments by senators stated that they were not against Burris as a person, but were against his appointment by Rod Blagojevich.

With the appointment, Burris becomes the only African-American member of the United States Senate. This makes the count of Democrats to Republicans in the Senate 58 Democrats and 41 Republicans, with the final seat waiting on a vote recount ordered by Norm Coleman in his loss to Al Franken of Minnesota.

U.S. military confirms Qur’ans were kicked, stepped on and splashed with urine at Guantanamo

On Friday, the U.S. military released the results of their investigation and confirmed that in 5 separate incidents, American guards at the Guantánamo Bay prison “mishandled” the Islamic holy book. However, they stress that guards were usually “respectful” of the Qur’an.

One incident involved splashing a Qur’an with urine by urinating near an air vent while others involved kicking, stepping on and writing in Qur’ans.

Brigadier-General Jay Hood, the commander of the jail, said that these incidents are the exception to the rule. In a statement issued late Friday, he said: “The inquiry … revealed a consistent, documented policy of respectful handling of the Qur’an dating back almost two-and-a-half years.”

Gen. Hood looked into the allegations, published and then retracted by Newsweek, that American personnel flushed a Qur’an down a toilet. He said that the inquiry did not find any evidence supporting this particular allegation. “The inquiry found no credible evidence that a member of the Joint Task Force at Guantanamo Bay ever flushed a Qur’an down a toilet. This matter is considered closed.”

The incidents reported are:

  • a guard kicking a prisoner’s Qur’an;
  • Qur’ans wetted by water balloons thrown by guards;
  • a “two-word obscenity” written, in English, inside the cover of a Qur’an. Military officials state that it is equally possible that a guard wrote this in the prisoner’s Qur’an or that the prisoner wrote this in his own Qur’an;
  • a guard who urinated too close to an air vent, spraying a Qur’an with urine;
  • an interrogator who stepped on a Qur’an during an interrogation.

The investigation also revealed 15 alleged cases of Qur’an mistreatment by detainees themselves. Detainees used Qur’ans as pillows, urinated on them, and, several times, tore pages out of copies of the books, according to the report. For example, the report states that a guard observed a “detainee place two Qur’ans in his toilet and state he no longer cared about the Qur’an or his religion,” on February 23, 2004. It is believed that such behavior is intended to cause disruption and problems for the guards.

New York executive files $60 million libel lawsuit over insurance scandal

A former Marsh & McLennan Cos. executive has hit former New York Governor Eliot Spitzer with a $60 million defamation lawsuit over an online magazine article regarding an insurance bid-rigging scandal.

William Gilman, a former Marsh managing director, filed a complaint last Friday in the U.S. District Court in Manhattan, over allegations Mr. Spitzer defamed him in a Slate article published a year ago. A copy of the complaint was made public on Monday.

Gilman, who had a final insurance fraud charge dismissed in January, said Spitzer acted with “actual malice” by suggesting that he was guilty of crimes of which he was never accused.

Although he wasn’t named in the article, Mr. Gilman complained that Spitzer defamed him by writing that “Marsh’s behavior was a blatant abuse of law and market power: price-fixing, bid-rigging and kickbacks all designed to harm their customers and the market while Marsh and its employees pocketed the increased fees and kickbacks.”

“While Mr. Spitzer’s statements do not refer to Mr. Gilman by name, Mr. Gilman is readily identifiable as the subject of the defamatory comments,” said the complaint. “Mr. Spitzer was well aware of his own allegations as attorney general and the resolution of those allegations in favor of Mr. Gilman and yet, recklessly disregarded these facts.”

In 2004 Mr. Spizter, then the state’s Attorney General, announced an investigation into the practices at Marsh & McLennan, particularly fees paid by insures to brokers who place business with them. Gilman, who worked for the company at the time, was charged in 2005 with 37 counts of insurance fraud. Gilman’s final charge was dropped last January.

“I haven’t seen the lawsuit and so will not comment on it,” said Spitzer. “The illegalities rampant at Marsh & McLennan leading to their fine of $850 million and the multiple judicial findings of illegality are clear from the public record.”

Mr. Gilman is now seeking at least $10 million in compensatory damages; $20 million in general damages, including damage to his reputation; and $30 million in punitive damages.

On Monday, at 10.14 pm UTC, the Wikimedia server cluster experienced a total power failure, taking down Wikipedia, Wikinews, and all other Wikimedia websites.

The servers are housed in a colocation facility (colo) in Tampa, Florida, USA. They occupy two racks, with each rack receiving electricity from two independent supplies. However, both supplies have circuit-breakers in them, and both opened at the same time, leading to a total power failure. All computers immediately went down. It’s normal for fire safety regulations to prohibit uninterruptible power supplies in colos, with the colo providing its own UPS and generator instead. The circuit breakers were on the computer side of this emergency power system, so none of the computers continued to receive power to survive the breaker trip or shut down safely.

The actual reason why the circuit breakers tripped is currently unknown.

When power was restored, it was discovered that most of the MySQL databases that store the data which makes up Wikipedia et al had been corrupted. The main database and the four slaves had all damaged the data on their hard disk drives beyond the ability of the auto-correction to repair. Only one copy survived safely, on a machine that is used for report generation and maintenance tasks, which remained 31 hours backlogged while catching up after an unusually heavy update load during the previous week.

Volunteer Wikimedia engineers worked through the night rebuilding the databases from the sole good copy onto the other servers. The Wikipedia database is over 180Gb in size, making the copying process last 1.5 – 2 hours for every server it was performed upon.

Regular back-ups of the database of Wikipedia projects are maintained – the encyclopedia in its entirety was not at risk. The last database download was made on February 9; all edits since then could only have been laboriously rebuilt from logs and recovered from the damaged database requiring much more time and effort.

Limited read-only service was established late Tuesday afternoon, with editing becoming possible 24 hours after the power failure. Final repairs continue now, as well as upgrades to prevent similar issues in the future. Server-intensive features, such as categories and ‘watchlists’ that display recent changes to selected articles to registered users, remain disabled to ease the load on the recovering systems.

The process which led to the damage originated with the operating system, disk controllers, or hard drives failing to flush the data correctly.

If the power to a database server is cut mid-write, the database may be corrupted and unreadable, however the operating system, hardware, and software are designed to make this very unlikely. In a previous incident in 2004 power was also lost to a server but the database was undamaged.

To avoid such damage, each database server saves a copy of an edit to be applied to the database on a separate storage system before making the actual update to the database itself. This so-called ‘write-ahead logging’ should ensure that in the event of a system crash, the database can be rebuilt from a ‘last-good’ state by replaying the edits saved in the log.

Earlier this year popular blogging site LiveJournal suffered a similar power failure when another customer at their colocation facility pressed an Emergency Power Off button, intended for use only by firefighters. The company suffered database corruption similar to that seen at Wikimedia.

LiveJournal are now fitting UPS to their servers to ensure that they have time to shut down safely in the event of a power failure. Wikimedia was said to be investigating the possibility of fitting similar equipment at the time of this failure.

Several pundits have suggested that the use of another database, such as the proprietary database Oracle or the free PostgreSQL, would have avoided the database corruption seen at the server cluster. A post-mortem of the incident show the failure was in the operating system, or the hardware, or some combination of the two. LiveJournal, which also uses MySQL, reported similar database corruption after their power cut.

The Wikimedia foundation only allows the use of free software on its systems, and future versions of the Mediawiki software will support the PostgreSQL database.

Users are reminded that during times of system failure or excessive demand, they can still search Wikipedia using Google. The articles may be viewed using Google’s cache.

Working in the medical sphere is a difficult and exacting job, and also one of the most valued occupations that a person can be employed in. Often, you will have to keep calm and carry on when most around you are not doing so. Whatever your job within the sector, people will look to you to live up to these exacting standards, and this is because the people for whom you are working are often recovering from a health scare which is putting them through hell. They don’t need any more worries, their sole focus must be to keep recovering until they can get a clean bill of health.

This kind of pressure can make any job tough, but it should also fortify you to know that you have been chosen to do this job by people who know you can do it. In that light, the best thing to do is simply get on with it. When you are on the job, there is not really any time to think about how overwhelming the job can be.

It seems bizarre to a lot of people that a patient might judge the efficiency of a doctor or nurse by the cut of their nursing scrubs uniforms, but it is actually understandable. You are looking after them, so you should be able to look after yourself. They are usually not in the best position to do it for themselves, regrettably.

After all, if you were getting over a serious illness and noticed that the person providing your front line care had a large rip or a noticeable stain on their scrub pants, would you feel reassured in your recovery? Most people would not, and it is easy to see their point of view when you really think about it.

Finding the best medical clothing at the most reasonable cost is, without a doubt, vitally important. Fancy scrub tops are not the issue, it is merely that the person wearing the top gives an impression of being trustworthy and in control. Even if in your mind you are nervous, exhausted and even a little scared, on the outside you need to be authoritative.

As long as your scrub jackets and other parts of your outfit are in decent shape, the patient will have more of a sense that you will ensure their comfortable recovery. This may sound like a crazy thing to say, but your job is not just about knowing medical procedure, it has a lot to do with presentation as well.

Cleveland, Ohio clinic performs US’s first face transplant

A team of eight transplant surgeons in Cleveland Clinic in Ohio, USA, led by reconstructive surgeon Dr. Maria Siemionow, age 58, have successfully performed the first almost total face transplant in the US, and the fourth globally, on a woman so horribly disfigured due to trauma, that cost her an eye. Two weeks ago Dr. Siemionow, in a 23-hour marathon surgery, replaced 80 percent of her face, by transplanting or grafting bone, nerve, blood vessels, muscles and skin harvested from a female donor’s cadaver.

The Clinic surgeons, in Wednesday’s news conference, described the details of the transplant but upon request, the team did not publish her name, age and cause of injury nor the donor’s identity. The patient’s family desired the reason for her transplant to remain confidential. The Los Angeles Times reported that the patient “had no upper jaw, nose, cheeks or lower eyelids and was unable to eat, talk, smile, smell or breathe on her own.” The clinic’s dermatology and plastic surgery chair, Francis Papay, described the nine hours phase of the procedure: “We transferred the skin, all the facial muscles in the upper face and mid-face, the upper lip, all of the nose, most of the sinuses around the nose, the upper jaw including the teeth, the facial nerve.” Thereafter, another team spent three hours sewing the woman’s blood vessels to that of the donor’s face to restore blood circulation, making the graft a success.

The New York Times reported that “three partial face transplants have been performed since 2005, two in France and one in China, all using facial tissue from a dead donor with permission from their families.” “Only the forehead, upper eyelids, lower lip, lower teeth and jaw are hers, the rest of her face comes from a cadaver; she could not eat on her own or breathe without a hole in her windpipe. About 77 square inches of tissue were transplanted from the donor,” it further described the details of the medical marvel. Leading bioethicist Arthur Caplan of the University of Pennsylvania withheld judgment on the Cleveland transplant amid grave concerns on the post-operation results. “The biggest ethical problem is dealing with failure — if your face rejects. It would be a living hell. If your face is falling off and you can’t eat and you can’t breathe and you’re suffering in a terrible manner that can’t be reversed, you need to put on the table assistance in dying. There are patients who can benefit tremendously from this. It’s great that it happened,” he said.

Dr Alex Clarke, of the Royal Free Hospital had praised the Clinic for its contribution to medicine. “It is a real step forward for people who have severe disfigurement and this operation has been done by a team who have really prepared and worked towards this for a number of years. These transplants have proven that the technical difficulties can be overcome and psychologically the patients are doing well. They have all have reacted positively and have begun to do things they were not able to before. All the things people thought were barriers to this kind of operations have been overcome,” she said.

The first partial face transplant surgery on a living human was performed on Isabelle Dinoire on November 27 2005, when she was 38, by Professor Bernard Devauchelle, assisted by Professor Jean-Michel Dubernard in Amiens, France. Her Labrador dog mauled her in May 2005. A triangle of face tissue including the nose and mouth was taken from a brain-dead female donor and grafted onto the patient. Scientists elsewhere have performed scalp and ear transplants. However, the claim is the first for a mouth and nose transplant. Experts say the mouth and nose are the most difficult parts of the face to transplant.

In 2004, the same Cleveland Clinic, became the first institution to approve this surgery and test it on cadavers. In October 2006, surgeon Peter Butler at London‘s Royal Free Hospital in the UK was given permission by the NHS ethics board to carry out a full face transplant. His team will select four adult patients (children cannot be selected due to concerns over consent), with operations being carried out at six month intervals. In March 2008, the treatment of 30-year-old neurofibromatosis victim Pascal Coler of France ended after having received what his doctors call the worlds first successful full face transplant.

Ethical concerns, psychological impact, problems relating to immunosuppression and consequences of technical failure have prevented teams from performing face transplant operations in the past, even though it has been technically possible to carry out such procedures for years.

Mr Iain Hutchison, of Barts and the London Hospital, warned of several problems with face transplants, such as blood vessels in the donated tissue clotting and immunosuppressants failing or increasing the patient’s risk of cancer. He also pointed out ethical issues with the fact that the procedure requires a “beating heart donor”. The transplant is carried out while the donor is brain dead, but still alive by use of a ventilator.

According to Stephen Wigmore, chair of British Transplantation Society’s ethics committee, it is unknown to what extent facial expressions will function in the long term. He said that it is not certain whether a patient could be left worse off in the case of a face transplant failing.

Mr Michael Earley, a member of the Royal College of Surgeon‘s facial transplantation working party, commented that if successful, the transplant would be “a major breakthrough in facial reconstruction” and “a major step forward for the facially disfigured.”

In Wednesday’s conference, Siemionow said “we know that there are so many patients there in their homes where they are hiding from society because they are afraid to walk to the grocery stores, they are afraid to go the the street.” “Our patient was called names and was humiliated. We very much hope that for this very special group of patients there is a hope that someday they will be able to go comfortably from their houses and enjoy the things we take for granted,” she added.

In response to the medical breakthrough, a British medical group led by Royal Free Hospital’s lead surgeon Dr Peter Butler, said they will finish the world’s first full face transplant within a year. “We hope to make an announcement about a full-face operation in the next 12 months. This latest operation shows how facial transplantation can help a particular group of the most severely facially injured people. These are people who would otherwise live a terrible twilight life, shut away from public gaze,” he said.
